Tywan D. Mata is a dedicated and passionate healthcare professional with a deep-rooted commitment to community service and improving the lives of others. Born and raised in the vibrant neighborhood of East New York, Tywan has always felt a strong connection to the community and a drive to make a positive impact.
Tywan’s journey in the healthcare field began in 2007 when he joined CAMBA’s Women’s Prison Association as a Case Planner. Working with incarcerated individuals, Tywan witnessed the importance of compassion, empathy, and support in the rehabilitation process. These early experiences fueled his desire to pursue a career centered around helping others and fostering positive change.
In 2011, Tywan transitioned to CAMBA’s Health Home Program, where he embarked on a journey of growth and responsibility. Over the years, he took on progressively challenging roles, starting as a Program Supervisor, then moving up to Program Supervisor II, and finally assuming the position of Program Director. In these capacities, Tywan honed his leadership skills, managing daily operations and collaborating with matrixed teams to ensure quality healthcare delivery. He felt privileged to serve as a point of contact for various stakeholders and contribute to the improvement of operational efficiency and communication.
Tywan’s dedication to community welfare extended beyond his professional roles. Since 2000, he has proudly served on the Board of Directors at Housing Works Health Care Inc., actively participating in shaping the organization’s mission and advocating for the needs of vulnerable populations. It brings Tywan immense satisfaction to contribute to the delivery of high-quality healthcare services and make a lasting impact on the lives of those in need.
In 2015, Tywan joined the Community Healthcare Network (CHN) as the Regional Deputy Director, overseeing the daily operations of CMA within the CHN Lead Health Home Program. Here, he continued to apply his skills in streamlining processes and implementing solutions to benefit both patients and staff.
In 2019, Tywan took on a new challenge and joined AIRnyc as the Director of Care Coordination. His dedication to patient-focused healthcare and strategic leadership soon led to his promotion to Chief of Care Coordination in 2021. This role allowed him to play a critical part in ensuring the continuity of caregiving during the trying times of the COVID-19 pandemic. By optimizing resources and fostering a positive work culture, Tywan aimed to enhance efficiency and stakeholder satisfaction.
Tywan’s commitment to professional growth led him to pursue higher education. He earned a Master of Social Work from Adelphi University in 2019, a Master of Public Administration with a focus on Public Affairs and Administration from Metropolitan College of New York in 2012, and a Bachelor of Science in Legal Studies (with a Minor in Psychology) from John Jay College of Criminal Justice, The City University of New York in 2007.
Outside of his professional endeavors, Tywan finds great joy in engaging in community service and giving back to the neighborhood. His unwavering commitment to community welfare drives him to make a positive impact on the lives of others, making Tywan a true asset to East New York and the broader community he holds dear.
